Hospice
HOSPICE: Assistance for terminally ill patients and families at home by a licensed nurse, nurse aides, volunteers, social workers and clergy members.
Medicare eligibility:
1. Care prescribed by physician
2. Diagnosed as terminally ill with six (6) months or less to live
3. Not receiving curative treatment
4. Primary caregiver available 24 hours/day
5. Committed to home care
6. Age - No minimum or maximum
Private Insurance eligibility:
1. Care prescribed by physician
2. Approval from insurance carrier
3. Age - No minimum or maximum
Private Pay eligibility:
1. Care prescribed by physician
2. Age - No minimum or maximum
