Caregiver Programs
- a system of support services to assist families and other informal caregivers of older individuals, and grandparents or older individuals who are relative caregivers of children.

    Information and Referral – one-on-one visitation occurs in the home setting and caregivers are assessed and given written information and resources for their area and needs

    Support and Training- volunteers are trained by a licensed nurse and then  provide one-on-one visitation with caregivers to provide caregiver support and training in the home

    Respite Care- provided in the adult day care and nursing home setting to provide temporary respite for the caregiver

    Support Group Meetings and Caregiver Training- local organizations hold support group and training meetings covering a variety of caregiver issues

     

Eligibility:

1. Age- 60+ for grandparent caregiver

2. Grandchild must be under the age of eighteen (18)

3. Caregiver of frail older individual is an individual unable to perform at least two (2) activities of daily living without human assistance due to cognitive or mental impairment

4. Caregiver must be providing in home and community care to the older individual

5. Income- no limitations
