Case Management
TARAGETED CASE MANAGEMENT: Provides assistance to Medicaid recipients in gaining access to needed medical, social, educational and other services provided under Arkansas Medicaid State Plan and other community services.
Eligibility:
1. Care prescribed by physician
2. Current Medicaid card
3. Have no available and reliable supports to provide assistance
4. Age 60+
SENIOR CASE MANAGEMENT: A private pay case management program to assist recipients and/ or family/friends with information and coordination of services.
Eligibility:
1. Approved for service by Area Agency on Aging nurse
2. State funded
3. Age 60+
CLIENT REPRESENTATION: Provides assistance to non-Medicaid recipients in gaining access to needed medical, social educational and other services available in the community.
Eligibility:
1. Approved for services by Area Agency on Aging nurse
2. State funded
3. Age 60+
