Home Health Services
SKILLED CARE: Home visits by a licensed nurse, nurse aides, physical, speech or occupational therapists and medical social worker for patients needing intermittent post-hospitalization care, wound care, injections, I.V. therapy, catheter care and other skilled nursing procedures that cannot by performed by a non-medical person.
Medicare eligibility:
1. Care prescribed by physician
2. Intermittent care prescribed by physician
3. Homebound
4. Be expected to improve
5. Medicare Part A or B
6. Age - No minimum or maximum
Medicaid eligibility:
1. Care prescribed by physician
2. Intermittent care prescribed by physician
3. Be expected to improve
4. Current Medicaid card
5. Age - No minimum or maximum
Private Insurance:
1. Care prescribed by physician
2. Approval from your insurance carrier
3. Age - No minimum or maximum
EXTENDED CARE: Medically necessary services provided by a licensed nurse under the direction of the recipient's physician in their place of residence. Example: Ventilator-dependent, Intravenous drug therapy, Hyperalimination.

3. Patient approval by Medicaid program
4. Prescribed by physician
