Personal care
PERSONAL CARE: Assistance with toileting, grooming, dressing, meal preparation, laundry and light housekeeping provided by nurse aides, prescribed by physician and supervised by registered nurse.
Medicaid eligibility:
1. Care prescribed by physician
2. Current Medicaid card
3. Require assistance with at least two (2) activities of daily living
4. Age - No minimum or maximum
Private eligibility:
1. Care prescribe by physician
2. Age - No minimum or maximum
Non Medicaid/State Funded eligibility:
1. Care prescribed by physician
2. Meet income guidelines
3. Require assistance with at least two (2) activities of daily living
4. Age 60+
